| Public Health-EMS EMT Certifications - Thursday, March 15, 2012Human Resources' Live Scan service is available for Public Health-EMS EMT certification purposes until May 15, 2012. The change coincides with the loss of services at CHA-HR in Riverside but is effective for all sites. After May 15, 2012, these Live Scans can be obtained at a public Live Scan provider. The service is available at these sites but some sites have certain restrictions:
- HR-Occupational Health, 14375 Nason Street, Suite 200, Moreno Valley, (951) 486-5531: The service here is limited to County employment applicants and RCRMC volunteer applicants.
- HR-Temporary Assignment Program, 1111 Spruce Street, Riverside, (951) 955-9178. The service here is for County employment applicant and volunteer applicant purposes. Until May 15, 2012, the service is also available for Public Health-EMS certification purposes.
- HR-Temporary Assignment Program, 44-199 Monroe Street, Suite B, Indio, (760) 863-2670: The service here is available on a bi-weekly basis and is for County employment applicant and volunteer applicant purposes. Until May 15, 2012, the service is also available for Public Health-EMS certification purposes.
- HR-Community Health Agency, 4065 County Circle Drive, Suite 206, Riverside, (951) 358-6137: Service here will terminate on May 15, 2012. Until May 15, the service is available for County volunteer applicant and Public Health-EMS certification purposes.

Live Scan for County Employment, Volunteers & Contractors

The forms on this page are for use by County Departments and Human Resources for Applicant Live Scan Services. Department Request Forms must be completed and submitted by the requesting department once the Live Scan is scheduled. Employment, Volunteer (except those volunteer types listed below), and Contractor applicant forms will be completed by the applicant at the time of their Live Scan appointment. Employee Live Scans must be performed by a County Human Resources Live Scan operator.
Volunteer Reserve Firefighter applicants and OES Disaster Corps Volunteer applicants must obtain the DOJ BCII 8016 Form directly from the Fire Department or OES, schedule their own Live Scan appointment, and bring the DOJ BCII 8016 Form to their appointment. These volunteers will complete all of the other forms (i.e., the Volunteer Declaration and the Background Check Disclosure, Release and Authoriation) at their Live Scan appointment. Fire/OES do not submit Department Request Forms. These Live Scans must be performed by a Human Resoruces Live Scan operator.
